Applicant must check with the Slovak embassy where he/she will be submitting his application and be sure of the availability of appointment and that they will accept application for visa before ordering this service.
Applicants living in the EU with type D visa or residence permit in other EU countries can apply.

REQUIRED DOCUMENTS
- Scanned copy of international passport valid for 1.6 years
- Two statement of account for 3 months with balance 5,700 Euro to support business registration and 3,500 Euro for each adult family member plus 1,750 Euro for each child under 18 years of age respectively.
- Marriage certificate and children birth certificate (for family)
- Police/Criminal Clearance Certificate – Must be translated to Slovak Language and
- One passport photograph on white background
